This template gives NOTES, MEMBER_RATING, TAGS and OPTIN_TIME no Salesforce field of their own.

The template writes them into Description, one labeled line each; a line with an empty cell is left out.

Mailchimp to Salesforce in three steps

Step 1Export from Mailchimp
  1. Click Audience, and choose the audience from the dropdown if you have more than one.
  2. Click Export audience.
  3. Click Export CSV and wait for the email saying the export is complete.
  4. Download the file. It may arrive as a ZIP with one CSV per contact status.

Only Owners and Admins can export audiences, and the files stay in your account for 30 days.

  3. Save Salesforce Leads import as a CSV.

Works in Excel for Microsoft 365 and Google Sheets. Excel 2019 and older, Apple Numbers and LibreOffice can't run the formulas this file uses.

Step 3Import into Salesforce
  1. From Setup, type Data Import Wizard in Quick Find, select it and click Launch Wizard.
  2. Under Standard Objects, choose Leads.
  3. Choose whether to add new records, update existing ones or both, and how to match them.
  4. Drag in your CSV, check the encoding and click Next.
  5. Check the mapping, map any field left over, click Next, then Start Import.

Questions

Where does Mailchimp's export fill Last Name and Company, which Salesforce requires?

Last Name comes from Mailchimp's Last Name column; Mailchimp's export has no column for Company, so add one before you import. The checker above counts the rows with Last Name blank.

Where do NOTES, MEMBER_RATING and the other columns go?

Into Description on each lead, labeled like "- Notes: Asked about the annual plan.".

What happens to unsubscribed contacts?

Mailchimp exports include all contacts regardless of subscription status. This template maps the contact fields but doesn't set email opt-out in Salesforce. Filter out unsubscribed rows before importing, or update the Do Not Email field in Salesforce after.
